Take your plate letter/email to a NYS DMV office to receive “TC” (TLC) For-Hire plates and registration. Notify TLC of your plate by filing the Plate Notification Form fouund on TLC UP. TLC will email and post on TLC UP your vehicle inspection appointment date.

Get a TLC Drivers License Minimum age: 19. Have a Valid DMV Chauffer's license- Class A, B, C, or E. You must have a valid Social Security Number. No fines or fees owed to NYS DMV or TLC. No more than 5 points on your DMV license within a 15 month period back from your most recent violation date.

To update your information on file with the TLC, you must submit a completed Change of Address Form in person at our Long Island City Facility located at: 31-00 47th Avenue, 3rd Floor, Long Island City, NY 11101.

You must file all the required documents online at nyc.gov/lars. Select 'apply for a new license' and select 'vehicle', then follow the instructions to upload the required documents. Once your application is processed you will be provided with a plate letter via TLC UP.

To submit, email a signed copy of this form to TLCcompliance@tlc.nyc.gov. You may also mail TLC a copy of this document when you mail your License to the TLC. Licenses can be mailed to the TLC at: TLC, 31-00 47th Avenue, 3rd Floor, Long Island City NY, 11101, Attention: Compliance Unit.

To change your name on your California driver's license or DMV identity card, you will need to submit a certified copy of a court-ordered name change and fill out a form DL-44. You can pick up a copy of the form at any DMV location (it is not available online).

TLC Driver License Replacement requests must be submitted in person, by making an appointment at the TLC's Long Island City facility. Appointments are made using TLC's online appointment system.
