List the owner or lessor name, purchase price, purchase date, and the number of axles. For buses, indicate the seating capacity. If the vehicle is a wrecker, check the applicable box.
- Complete Section D by providing the apportioned account number and the signature of the applicant or authorized signer. Include their title, date, and telephone number.
