1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Fill in the 'Type of Event' field, selecting the nature of your event, such as 'Fire Extinguisher Training'.
3. In the 'Requested Date/Time' section, provide the exact date and time for your event. Ensure that your request is made at least two weeks in advance.
4. Enter the 'Location' of the event, including street address and city, for clarity.
5. Complete the 'Group Name' section by including the name of the organization hosting the event.
6. Specify the 'Audience Age/Grade' so the Fire Department can tailor the training appropriately.
7. Input the 'Event Name' with a clear indication, for example, 'Parks and Rec extinguisher training'.
8. Provide an 'Alternate Date/Time' option in case your first choice cannot be accommodated.
9. Estimate and fill in the 'Projected Attendance' to give the Fire Department an idea of how many participants to expect.
10. In the 'Contact Person' section, list the name, email, and phone number of the individual responsible for the event.
11. Detail the 'Reason for Request / Details of the Event' to outline the purpose of the training.
12. Include any 'Additional Comments' that may benefit the Fire Department in processing your request.
13. Review all filled sections to ensure accuracy and completeness before finalizing.
14. Once you are satisfied with the information entered, save your changes, and if needed, download or print the request form for your records.
15. Submit your request via email to the provided contact address.
