6. Fill in the training program information. If you are unsure about this section, contact the training company for details such as the program sponsor and date of examination.
7. Attach a copy of your food manager’s certificate as required, and include the payment of $25. Acceptable payment methods are a business check, cashier’s check, or money order. Personal checks will not be accepted.
8. If you are requesting a duplicate certificate for a lost or damaged certificate, ensure to indicate that in your application, and include the certificate number along with the appropriate fee of $12.50.
9. Review all filled-in details to ensure accuracy before finalizing your application. Once completed, save your changes and choose to download, print, or share the application as needed.
