All applicants must fill out and sign the Application for Disabled Person Placard [dor.mo.gov] form 2769. If you are the applicant and apply in person at a Missouri Department of Revenue Office, the office can assist you in filling out the form.

Pay the substitute placard fee (temporary placards only; DMV does not charge a fee to replace permanent and travel placards). Make an appointment and bring your completed application to a DMV office near you, or send by mail.

There is no fee required to obtain a Permanent Disabled Placard, and the placard expires on September 30th of the fourth year from which the placard was issued.

Please allow 45-60 days to process your application/certification form. If you are applying for a temporary disability parking placard (good for up to six months depending on your diagnosis), you may visit a Secretary of State facility and be issued a red, temporary disability parking placard.

Once you have a fully licensed and valid plate or placard on your vehicle, you are now free to park in any handicap space that is open.

Submit a copy of the original application, along with the new application to your county tax office. If you do not have a copy of your application, you may also submit your expiring placard(s) to your county tax office.

Applicants must complete the Application for Disabled Person Placard (Form 2769). The applicant's licensed physician, physician's assistant, podiatrist, chiropractor, advanced practice registered nurse, or optometrist must also complete a Physician's Statement for Disabled Person's Plates/Placard (Form 1776).

You can get the Application for Disabled Person Placard or Plates (Form REF 195) by going to your local California Department of Motor Vehicles (DMV) Office or printing the document online. There is no fee for a permanent parking placards and license plates, but there is a $6 fee for a temporary parking placards.
