4. For a social security number change, state the incorrect number and the new number you wish to be recorded. Original documentation, such as a social security card, is required for submission at a campus A&R office.
5. To change your date of birth, provide the correct date and include original documentation. This change also requires submission at a campus A&R office.
6. For a change of address, enter your new street address, city, state, zip code, and phone numbers. Ensure that you include proof of your new in-district address as specified on the back of the form.
7. Review the form for accuracy before you proceed. Once completed, save your changes, and prepare to download or print the form.
8. Submit the completed form either online, or by mailing it to the ACC Admissions & Records, or visiting a campus A&R office. Be sure to include a copy of a photo ID if mailing or faxing the request.
