You (the Borrower) can apply for forgiveness of your Paycheck Protection Program (PPP) loan using this SBA Form 3508S only if the total PPP loan amount you received from your Lender was $50,000 or less.

After receiving a PPP loan, borrowers must apply for forgiveness through their lender by filling out the Small Business Administration (SBA) form and providing supporting documents. SBA Form 3508 is the document PPP loan borrowers must complete to request loan forgiveness.

SBA Form 2483 (3/21) Paycheck Protection Program. Borrower Application Form Revised March 18, 2021. Purpose of this form: This form is to be completed by the authorized representative of the Applicant and submitted to your SBA Participating Lender.

While anyone who is self-employed, an independent contractor, or a sole proprietor who had no employees when they applied for a PPP loan is eligible to use the EZ form, it gets more complicated from there. If you reduced employees' pay by more than 25%, use the regular 3508 form.

A BORROWER MAY USE THIS FORM ONLY IF THE BORROWER RECEIVED A PPP LOAN OF $50,000 OR LESS. A Borrower that, together with its affiliates, received PPP loans totaling $2 million or greater cannot use this form.

Requirements include: Copies of canceled checks. Bank statements with ACH information. Utility bills. Mortgage statements. Lease agreements. List of all employees on your payroll during the covered period. Evidence that your employees were kept on payroll or rehired upon receiving the loan.

An Applicant that files an IRS Form 1040, Schedule C, and elects to calculate the PPP loan amount using net profit must use SBA Form 2483-SD. An Applicant that files an IRS Form 1040, Schedule F, and calculates the PPP loan amount using gross income must use SBA Form 2483-SD.

We want to clarify that completion of Form 3508D is only required in certain circumstances which we are outlining below. It is not required for all PPP borrowers. The Form 3508D is mentioned in a section that refers to a specific situation as outlined below. It only needs to be completed when the following applies.
