You may request military service records (including DD 214) if you are: A military veteran, or. Next of kin of a deceased, former member of the military. The next of kin can be any of the following: Surviving spouse who has not remarried. Father. Mother. Son. Daughter. Sister. Brother.

If you do not have an original DD 214, a certified copy can be requested online at .archives.gov/veterans/military-service-records or submitted by mail or fax using the SF-180 Form which is provided on the same website.

Many civilian employers are aware of the long form, including discharge code designations, and may ask for a copy during the hiring process. If applying for a government position, either municipal, state or federal, veterans should expect to be asked to provide a copy of their DD-214.
