8. For Section 7, Disability Information, confirm if you have a disability. If yes, you may need to submit a Disability Certification Form completed by a healthcare professional.
9. In Section 8, Photograph, if you do not have a photo on your swipe card, include a recent wallet or passport-sized photograph.
10. Complete Section 9 with your signature and date. This confirms that all information provided is accurate and confidential.
