1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Fill out your personal information in the Employee Information section, including your legal name, address, phone number, and email address. Ensure all details are correctly inputted.
3. Complete the Employee Confidentiality Privacy Information Agreement by acknowledging your understanding of HIPAA privacy regulations. Read the agreement and sign it.
4. Fill out the Relationship Disclosure Form if applicable. Indicate your relationship with your employer and sign the form.
5. Authorizations are required for background checks. Complete the Employee Authorization to Perform Background Check(s) and provide your past addresses if you have lived in Pennsylvania for less than five years.
6. Complete the Form I-9 for Employment Eligibility Verification. Fill in your information in Section 1 and confirm your citizenship or immigration status.
7. Fill out Federal Tax Withholding (Form W-4) to determine the correct amount of federal income tax to withhold from your pay.
8. Complete the Residency Certification Form (Form CLGS-32-6) to report your residence for local earned income tax withholding.
9. If you choose Direct Deposit, fill out the Direct Deposit Agreement Form with your banking details.
10. Review all forms to ensure that all sections are completed accurately. Once verified, save your changes, download the forms, print, or share as required.
