- Enter the date of signing and details for the witness, including their name, address, qualification, mobile number, and phone number.
- Once completed, you can save any changes made to the form. You may also choose to download, print, or share the form as necessary.
- Finally, submit your completed form by emailing it to pensions@csc.gov.au or posting it to GPO Box 2252, Canberra ACT 2601.
