6. If you are withdrawing from CAP, record your child’s last day of attendance and the reason for withdrawal in the designated area.
7. For credit card changes, you cannot write the complete bank card number on the form. Instead, ensure it is securely stored in your online account. Enter the last four digits of your new bank card in the specified field.
8. Complete the fields for the cardholder’s name and the CVC code from the back of your card.
9. Once all fields are filled out, review the form for accuracy. You can save changes, download, or print the form as needed.
