5. In the 'Your Information' section, fill out your name, address, city, state, zip code, phone number, and email. Indicate if your address or phone number has changed by responding 'Yes' or 'No'.
6. Review the authorization statement regarding your pet’s medical records and ensure that all entered information is true and accurate.
