A 'Health Home' is not a place; it is a group of health care and service providers working together to make sure you get the care and services you need to stay healthy. Once you are enrolled in a Health Home, you will have your own care manager.

That's about $1150 a week for a three-person family, about $1387 a week for a family of four. Families with somewhat higher incomes pay a monthly premium of $15, $30, $45, or $60 per child per month, depending on their income and family size. For larger families, the monthly fee is capped at three children.

Two behavioral health homes have been designated to serve Westchester County. They are: Hudson River Healthcare, Inc. and Open Door Family Medical Center Inc./Hudson Valley Care Coalition.

Who is Eligible to be in a Children's Health Home? Children from birth to age 21 must be enrolled in Medicaid AND meet eligibility criteria by having two or more chronic health conditions (list of chronic conditions) OR one of the following single qualifying conditions: HIV/AIDS. Serious Emotional Disturbance.

An individual must be assessed and found to have significant behavioral, medical, or social risk factors to deem them appropriate for Health Home services. Appropriateness for Health Home services must be determined for list assigned members, MAPP HHTS Referral Portal referrals, as well as community referrals.

New York State has a health insurance plan for kids, called Child Health Plus. Depending on your family's income, your child may be eligible to join either Children's Medicaid or Child Health Plus.
