Filling out the Arizona Department of Revenue Form 322, which is used for claiming credits for contributions made or fees paid to public schools, can be straightforward with the right guidance. This guide will walk you through each section of the form, ensuring that you provide the necessary information to successfully complete your submission.
Follow the steps to complete the form online.
- Click 'Get Form' button to obtain the form and open it in your chosen document management system.
- Fill in your name as shown on Form 140, 140NR, 140PY or 140X, as well as your Social Security number. If filing jointly, include your partner’s name and Social Security number.
- In Part 1, Current Year’s Credit, report cash contributions made or fees paid to public schools from January 1, 2021, through December 31, 2021. For each contribution, provide the date, the CTDS code, the name of the public school, the school district name or charter holder name, and the amount contributed.
- If you contributed to more than three public schools, complete the Continuation Sheet on page 3 and include it with your form. Ensure to report the total contributions made to any additional schools on the appropriate lines.
- Total the contributions made during 2021 by adding the amounts reported in column (e) and indicate that total on line 5.
- Continue to Part B to report any cash contributions made or fees paid from January 1, 2022, through April 18, 2022. Repeat the process of providing details for each contribution just as in previous steps.
- Compute the total credits from both parts. Enter the smaller amount between line 11 or line 12 on line 13 to determine the total current year’s credit.
- In Part 2, document any available credit carryover from previous tax years. Complete the required columns to show the credit amounts and calculate the available carryover.
- In Part 3, summarize the income tax credits by listing the amounts from previous parts. Add the current year's credit and any carryover credit to find the total available credit.
- Once you have completed all sections, you can save your changes, download the form, print it, or share it for submission.
