6. In Section 5, disclose income information. Indicate whether you or any household members have received income in the past 30 days, including the source and total amount.
7. Review and complete Sections 6 and 7, where both the Head of Household and the community support worker must certify their understanding and agreement to the program terms.
8. Attach the required documents: Verification of Disability (Attachment A), Permanent Housing Plan (Attachment B), and Authorization for Disclosure of Consumer Medical/Health Information (Attachment C). Ensure all attachments are completed and signed as necessary.
9. Prepare a detailed letter on agency letterhead by the community support worker, describing the nature of the housing crisis and clarifying the Head of Household's situation and resources.
