5. In Section E, fill out the CPD points claimed for work-based activities for the last three years. Ensure to indicate the dates employed, employer/company name, and points claimed for each year.

7. After completing all sections, double-check that all evidence is attached and clearly marked. Remember to sign the declaration section on the first page.
