- Finally, both the employee and evaluator should sign the Performance Review Acknowledgment section, confirming that the review has been discussed. Make sure to also obtain the practice owner’s signature.
- Once all sections are filled out and signed, you can save the changes, download, print, or share the completed performance review form.
