7. Attach a medical certificate that details the child's disability and percentage. Ensure it is countersigned by the SMO of the local Air Force, Army, or Navy unit.
8. Review all information for accuracy. Sign and date the form in the designated spaces for the parent or guardian. The form needs to be countersigned by the Station Commander of the nearest Air Force unit.
