- Sign the application to authorize the release of your transcripts. Please note that there is a fee of $10.00 for each copy requested.
- Ensure that you have completed the checklist of required forms and documentation, including the request form itself, a signed authorization form if releasing to a third party, and a copy of your photo identification, as well as that of the third party if applicable.
