- If the student is participating in subsequent sports in the same school year, complete Section 5. This re-certification ensures any changes in health status are updated. The parent or guardian answers supplemental health questions again and submits this section to the Principal or designee.
- In Section 6, a licensed physician must certify if the student is fit or if there are any conditions that might limit participation. The physician should review previous sections and sign with their approval.
- Finally, if the student is participating in wrestling, complete Section 7 which involves establishing and certifying the minimum wrestling weight. This must be done by an AME along with relevant assessments.
