- Fill out the Loan Estimate. If it is not from UWM, ensure the lender name is left blank.
- Complete the Fee Itemization Form.
- Include a 4506-T Transcript of Tax Return.
- Sign the Borrower’s Certification and Authorization.
- Review and attach the Privacy Policy.
- Ensure that all State Specific Disclosures are included as required.
