4. In Section 3: Certification/Authorization for Check Stop Payment, indicate whether the payment instrument was lost, stolen, or damaged/destroyed. Certify that you have not cashed or used the payment instrument and authorize the stop payment request. Provide your signature, date of request, contact email address, and contact telephone number.
5. Finally, complete Section 4: Replacement Check Mailing Instructions by indicating where you want the replacement check mailed. You have the options of your home address on file, mailing address on file, or other.
6. After filling out all sections thoroughly, save the completed form. Ensure to review your entries for accuracy, then submit your completed form for processing.
