If you don’t see your employee ID on your pay statement, enter your company code and employee file number, which can be found at the top left corner of your pay statement.
- Move on to Section 2: Check Detail for Stop Payment. Enter the check number, original pay date, net check amount, and company name as detailed on your pay statement. If you need assistance with these details, you can retrieve them from iPay or contact the Employee Service Center.
- In Section 3: Certification/Authorization for Check Stop Payment, indicate whether the payment instrument was lost, stolen, or damaged/destroyed. Certify that you have not cashed or used the payment instrument and authorize the stop payment request. Provide your signature, date of request, contact email address, and contact telephone number.
- Finally, complete Section 4: Replacement Check Mailing Instructions by indicating where you want the replacement check mailed. You have the options of your home address on file, mailing address on file, or other.
- After filling out all sections thoroughly, save the completed form. Ensure to review your entries for accuracy, then submit your completed form for processing.
