4. Indicate the conditions for your continuation by checking the appropriate options provided. Fill in any applicable amounts where specified, such as 'raising my pay to Rs. ___________________'.
5. In the 'Existing Pay Band and Grade Pay' section, clearly write your current pay band and grade pay in the designated space.
6. Proceed to sign the form in the signature box and provide your name, designation, and the office in which you are employed.
7. Complete the Undertaking section by signing again and filling in your name and designation. Ensure you understand that any excess payment made against rules must be refunded.
8. Once you have filled out all necessary sections, review the form for accuracy. Then, save your changes, download, print, or share the form as needed.
