Follow the steps to accurately complete your application

3. Provide your PORAC ID number if available. If you do not have a PORAC ID number, include a clear copy of the front and back of your Retired ID Card.
4. Indicate your retirement date and type of retirement, along with your agency and association name, spelled out completely.
5. Specify your job classification at the time of retirement and indicate whether you were a member of the association at that time, checking 'Yes' or 'No'.
6. If applicable, confirm if you intend to join the PORAC Anthem Blue Cross Medical Plan by selecting 'Yes' or 'No', and follow through with the necessary submission of both applications.
7. Select your method of payment. Include either a check payable to 'PORAC', or credit card information with the card number, expiration date, security code, and name as it appears on the card.
8. Provide your Social Security number for CalPERS deductions (if applicable), ensuring accuracy as this information is necessary for processing.
9. Sign and date the application to confirm that all provided information is accurate.
10. Review the entire form for completeness and accuracy, then save your changes and proceed to download or print the completed application.
