Follow the steps to complete the PSC-35 scoring sheet online

Pediatric Symptom Checklist (PSC-35)

The PSC consists of 35 items that are rated as “Never,” “Sometimes,” or “Often” present and scored 0, 1, and 2, respectively. The total score is calculat- ed by adding together the score for each of the 35 items.

Subscale Analysis: The PSC-35 contains three subscales that can add nuance to the over-all results. The Attention subscale is derived from adding items, 4, 7, 8, 9, and 14. The Anxiety and Depression subscale is derived from adding items 11,13, 19, 22 and 27.

The total score is calculated by adding the 35 individual scores, so the total score will be 0 to 70. If one to three items are left blank, they are ignored (and given a score of 0). If four or more items are left blank, the questionnaire is considered invalid.

What is the PSC–17? The Pediatric Symptom Checklist (PSC) is brief screening questionnaire that is used to improve the recognition and treatment of psychosocial problems in children ages 4 to 17 years. It is used in place of the PHQ-9 for Health Home beneficiaries within this age group.

Parents/caregivers will complete the PSC-35 (parent-completed version) for their children ages 3 and youth up to age 18. The PSC-35 will need to be completed at the beginning of treatment, every six months following the first administration, and at the end of treatment.

The PSC-35 total score is the sum of the 35 items [range 0-70]. See scoring sheet for instructions on scoring the domains. The measure is considered invalid if 4 or more items total are missing. For valid measures with missing items, missing items are scored as 0.
