- Start by entering the hospital name and the date at the top of the form. Providing this information is crucial for tracking and reference.
- Fill in the patient’s name in the designated field. Ensure accuracy in spelling to maintain proper records.
- Notate the time of the 12 lead ECG completion and hand it to the ED physician. Record this time in the appropriate section.
- Notify the charge nurse of the STEMI alert by checking the box or writing a note in the comments section, indicating notification.
- Document the time of continuous pain or symptom onset in the specified area, and ensure this information is reported to the STEMI scribe.
- List medications administered (Oxygen at 2 liters NC, ASA, , IV bolus, and/or Sulfate or ), following physician orders. Provide details regarding any contraindications, such as nitrate administration.
- Indicate IV access setup and specify the saline IV running at the Keep Vein Open (KVO) rate. If additional IVs are set up, repeat this step for each.
- Prepare the patient for transport by following the outlined procedures, including clothing removal and documentation of clothing in a bag.
- Facilitate the patient's transport and ensure that transfer paperwork is thoroughly completed for the receiving facility.
- Record EMS arrival and departure times in the designated data sheet areas.
- Send the completed data sheet and ECG with the patient during transfer to DHMC, ensuring all information is clearly noted.
- Call the report to DHMC Cath lab, being mindful of after-hours processes if applicable.
- After the STEMI alert procedure is complete, return the form to the STEMI scribe for inclusion in the RED STEMI ALERT PACKET.
- Use the comments section to provide feedback on the alert process, noting any successes or areas for improvement.

For optimal results, fibrinolytic therapy should be administered as early as possible, preferably within the first 3 to 6 hours and potentially up to 12 hours after the onset of symptoms (Figure I in the Data Supplement).
