For optimal results, fibrinolytic therapy should be administered as early as possible, preferably within the first 3 to 6 hours and potentially up to 12 hours after the onset of symptoms (Figure I in the Data Supplement).

For patients who've had an ST-elevation myocardial infarction (STEMI), the standard of care is myocardial reperfusion using percutaneous coronary intervention (PCI) or thrombolytic agents. PCI is preferred, and time is of the essence.

In STEMI give: Aspirin 300 mg po then 100 – 150 mg daily thereafter.... The ideal treatment for all STEMIs is acute PCI or in some cases emergency CABGs. Primary PCI is preferred for reperfusion therapy in patients with STEMI if it can be performed within 90 minutes of first medical contact.
