11. State whether you are physically impaired and briefly specify the type, if applicable.
12. Confirm whether you passed your exam from the Maharashtra board by selecting Yes or No; if No, provide the name of the board.
13. Fill in the name of your school, junior college, or college, along with your seat number, month and year of passing, percentage, and class or grade.
14. If applicable, provide details about any educational gaps.
15. Indicate whether you belong to a minority; if yes, specify the type.
16. Sign the declaration at the end of the form to confirm the accuracy of your information.
17. After completing the form, save your changes and choose to download, print, or share the form as needed.
