ONE IS NOT QUALIFIED TO SERVE AS A JUROR IF: If you are not an American Citizen. If you are not a resident of Queens County. If you have served on jury duty within the past 4 years. If you have been convicted of a felony.

Any jurors 70 years of age or older will be excused from jury duty at their request. Women who are currently breastfeeding will also be excused upon request. If there are other circumstances in your life that would make serving on jury duty an undue hardship, you may request to be excused from jury duty.

If you don't complete a jury form, likely you will be called for jury duty anyway. If you have not filled out the jury form, you are usually directed to do so by the bailiff when you arrive. If you refuse to complete the form, you may be held in contempt of court by the judge.

Yes; failure to respond to a jury questionnaire could result in fines or other penalties. You should respond by visiting the court in person and showing the clerk your passport, or by mailing in the form with a copy of your passport.

The Juror Qualification Questionnaire simply allows the Court to determine if a person is qualified to serve as a juror. If a person is selected to serve and receives a summons to report, he/she may ask to be excused at that time.
