Follow the steps to complete your membership application.

1. Click ‘Get Form’ button to access the application form and open it in your preferred editor.
2. Begin by entering your personal information. This includes your name, home address, city, zip code, home phone, cellular phone, pager number, and email address.
3. Next, provide your organizational information. Fill in your rank or title, organization name, office address, city, and zip code. Indicate your organization status.
4. Answer some additional questions. These include your availability for deployment, whether your jurisdiction has an approved IEMMAS agreement, your membership status with IESMA, any felony convictions, and your proficiency with computer software. Please note that you must also provide a copy of your valid Illinois driver’s license and amateur radio license with your application.
5. Detail your education background. Include your high school or general education degree completion date and any college or university experience, areas of study, years attended, and degrees or certifications received.
6. Describe your emergency management experience. Include the relevant experience and explain why you believe you would be a good addition to the IESMA-EMAT.
7. List any special skills you possess that could benefit the IESMA-EMAT.
8. Address the training requirements. For each relevant course, provide the date completed. Be sure to include copies of your EMI and IEMA transcripts.
9. Provide professional references as required. For each reference, include their name, home address, city, zip code, home phone, cellular phone, pager number, other contact information, and email address.
10. Complete the required signature authorizations section. Ensure to sign with your printed name, along with the employer's and chief elected official's necessary authorization and contact information.
11. Upon completion, save your changes and consider downloading, printing, or sharing the form as needed before submitting it to the EMAT Coordinator.
