 - Ensure each principal owner, member, or corporate officer completes the affidavit section, providing their legal name, ID number, social security number, birth date, and residential address.
 - Notarize each affidavit of signature. All documents must be signed and cannot contain whiteout or alterations.
 - Submit the completed application, along with the required documentation and the application fee of $117.90, either online or via mail.
