7. In the previous employment section, provide details for up to three past employers. Include the company name, phone number, address, supervisor’s name, your job title, starting and ending salaries, key responsibilities, the dates you worked, and the reason for leaving. Indicate whether you permit contact with each previous supervisor.
8. If applicable, fill out the military service section with the branch you served in, dates of service, rank at discharge, type of discharge, and any explanations needed.
9. Read and agree to the disclaimer and signature section by checking that all answers are true and complete. Sign and date the application.
