- For learners in senior high school, indicate the track and strand if any.
- Complete the certification statement by ensuring all provided information is accurate, and proceed to sign over the printed name of the parent or guardian, including the date.
- If required, the class adviser will fill in additional information under the 'For use of DepEd Personnel Only' section, including the date of first attendance, grade level, and track for senior high school.
