How to fill out the DBA Business License - Winnebago County Clerk online
The DBA Business License, administered by the Winnebago County Clerk’s office, is essential for sole proprietorships and partnerships operating under an assumed name. This guide will provide clear, step-by-step instructions on completing the online form for obtaining this important business license.
Follow the steps to successfully complete your DBA Business License application
- Click ‘Get Form’ button to obtain the DBA Business License form and open it in the form editor.
- Fill out the ‘Assumed Name Certificate of Intention’ form in triplicate, ensuring to use black ink throughout.
- If you complete the form prior to visiting the County Clerk's office, ensure to have your signature notarized before submission. Should you fill it out at the office, sign in the presence of a deputy clerk who will then affix the official seal.
- Bring a valid driver’s license or state ID for identification as you submit your form.
- Prepare to pay a $5.00 filing fee when you submit the ‘Assumed Name Certificate of Intention’.
- After filing, you will receive a dated receipt for your payment along with two copies of the submitted form for your records.
- Take the receipt and the two copies to a local newspaper that publishes at least weekly and has a legal notice section. The notice of your assumed name must appear there within 15 days of initial filing.
- The newspaper will publish your notice once a week for three consecutive weeks. Please note that the publication fee is your responsibility as the business owner.
- After the publication period ends, follow up with the newspaper to ensure they send the Certification of Publication to the Winnebago County Clerk's Office.
- Once the County Clerk's office receives the certification, they will issue your Assumed Name ‘Certificate of Ownership’, confirming your business registration in Winnebago County.
- Do not start operating under the assumed name until you have received the Certificate of Ownership from the Clerk's office.
- In the event of any changes related to ownership, addresses of owners, or the business itself, be sure to file the necessary updates with the County Clerk’s office.
