- Indicate the type of residence for Applicant 1 and provide their contact numbers, including home, work, and mobile, along with an email address. Confirm their permanent New Zealand residency status.
- If the child is under 13 years old, complete the Applicant 2 (Parent/Guardian) section with similar details: surname, first name(s), home address, gender, date of birth, occupation, and contact details.
- Again, include the IRD number, type of residence, and confirm residency status for the Parent/Guardian section as well.
- In the ASB FastCash card, FastPhone, and FastNet Classic section, select preferences regarding access to these services. Remember, these services are not accessible to parents/guardians for the Head Start account.
- Review the Account Conditions. Acknowledge the terms and conditions by confirming your understanding of them.
- Complete the Personal Declaration, ensuring all information provided to ASB is true and correct.
- Once all sections are filled out, review your information for accuracy before saving changes, downloading, printing, or sharing the completed form.
