8. Complete Part C(4) if you are applying for a substance abuse treatment clinician, detailing the percentage of work related to substance abuse treatment.
9. If you have temporary staffing needs, complete Part C(5) to explain how this position will replace locum tenens staff.
10. Once all sections are accurately completed, print the attestation page, sign it, and scan it for submission.
11. Attach the completed application and any required evidentiary documents to a single email and send it to SHARP.Applications@alaska.gov with the subject line ‘SHARP Employer Application [Employer Name]’.
12. Ensure that both Sections I and II are submitted to complete the application process.
