With regard to reporting to Cal/OSHA, a serious injury or illness is now defined as one involving. inpatient hospitalization, regardless of length of time, for other than medical observation or diagnostic testing; amputation; loss of an eye; or. serious degree of permanent disfigurement.

any reportable death, injury, occupational disease or dangerous occurrence. all work-related injuries that result in a worker being away from work or unable to do their full range of normal duties for more than three consecutive days (not counting the day of the accident but including any weekends or other rest days)

Employers must immediately report to Cal/OSHA any work-related death or serious injury or illness. We encourage employers to do so by telephone, 24 hours a day, 7 days a week: In what type of industry did the accident occur?

An effective incident investigation process involves four key steps: incident description, identification of causes, identification of changes to the organisation and work processes, and learning.

(a) Basic requirement. You must record on the Cal/OSHA Form 300 the recordable injuries and illnesses of all employees on your payroll, whether they are labor, executive, hourly, salary, part-time, seasonal, or migrant workers.

All employers are required to notify OSHA when an employee is killed on the job or suffers a work-related hospitalization, amputation, or loss of an eye. A fatality must be reported within 8 hours. An in-patient hospitalization, amputation, or eye loss must be reported within 24 hours.

Cal/OSHA regulations require that employers must report any Serious Injury/Illness or Fatality to the nearest Cal /OSHA District Office. practically possible but not longer than 8 hours after the employer knows or with diligent inquiry would have known of the serious injury or illness.

Every work-related serious injury or illness or fatality to County employees must be reported promptly and no later than 8 hours to the nearest Cal/OSHA district office of the Division of Occupational Safety and Health (DOSH).
