- Optionally, provide the employer's contact details in Boxes 11 and 12.
- Once the employer information is complete, move on to the employee information section. In Box 14, input the employee's Social Security Number (SSN). Box 15 allows you to optional enter the employee’s first day of work.
- Fill out the employee’s first name in Box 16, middle name in Box 17, and last name in Box 18.
- In Box 19, enter the employee's home address, followed by the city, state, and ZIP code in Boxes 20, 21, and 22.
- If the employee is located outside the United States, provide the province/region, country, and postal code in Boxes 23, 24, and 25 respectively.
- Optionally, include the employee's date of birth in Box 27 and salary details in Box 28. In Box 29, select the salary frequency by checking the appropriate box.
- Once all required fields are completed, review the form for accuracy. You can then save any changes made, download the form, print it, or share it as necessary.
