Fill in their contact number, email address, and mailing address, ensuring accuracy as all correspondence and results will be sent to this address.
5. Read and acknowledge the Affidavit Declaration in Part D. This declaration requires the representative to confirm that all information is true and complete. They must agree to various conditions regarding the test and the management of the application details.
6. Ensure that the affidavit is duly signed and dated by the representative. By ticking the acknowledgment box, the candidate and the representative confirm their agreement to the affidavit.
7. Review all provided information for accuracy. Once everything has been checked, you can save the changes, download a copy of the form, print it if necessary, or share it as required.
