- Fill in Column C with the Purchase Request Number, Procurement Request Order Number, or Military Interdepartmental Purchase Request Number, as applicable.
- In Column E, indicate the quantity of items scheduled for the report period on line C and the actual quantity delivered on line D.
- Report cumulative totals in Column F, using line C for the total scheduled and line D for the total delivered up to the report period's end.
- In Columns G through N, provide quantities scheduled and best estimates for future deliveries for the item.
- In Column O, state the remaining balance of contract quantities not previously indicated.
- Proceed to Section II for Delay Factors. If delays exist, detail them accordingly, if none, place an 'X' in the designated box.
