- Begin with the ITP section. Here, indicate the planning for the student's transition to post-secondary life, including education, employment, and independent living skills.
- Move to the Agency Notification section. List any agencies that will provide services to the student and how they will be notified.
- Proceed to the Iep GSI Page 1. Fill in the general student information, such as name, date of birth, and grade level.
- Continue to Iep GSI Page 2. Provide detailed information about the student’s strengths, needs, and goals.
- Fill out the Iep Instruction Plan Page(s) by outlining the specific instructional strategies and services that will be used to meet the student’s needs.
- Complete the Iep Accommodation Page, documenting any accommodations necessary for the student to access learning.
- On the Iep Program Service Page, describe the program services that will be offered to the student.
- Fill out the Iep Services/Placement Page, specifying the educational placement and any related services.
- Accurate information should be provided on the Iep Placement Page regarding the actual setting where services will be delivered.
- Ensure the Documentation of Receipt of Modifications/Accommodations and/or Behavior Management Plan Page is filled, acknowledging any modifications or behavior plans in place.
- Complete the Assistive Technology Page, even if the student does not require assistive technology, to document the consideration of such tools.
- Use the Consent to Bill Medicaid Letter section to gain necessary parental consent for billing Medicaid for services.
- Provide necessary information in the Prior Notice Page(s) regarding notifications to parents about services and changes.
- If applicable, fill out the Excusal Request Form, indicating any requests for participants to be excused from specific meetings.
- Complete the LAAI Form and LAA2 Form if needed, detailing specific assessments.
- Fill out the Transitions Pathway Signature Page to document the agreed-upon transition services.
- If applicable, complete the Behavior Management Plan to address any behavioral needs.
- Include the Age of Majority Parent and Age of Majority Student sections, noting any relevant information about student rights.
- Lastly, if necessary, fill out the IHP (Individualized Health Plan) to outline health-related needs.
