- Answer whether the Dance Hall was previously operated under another name and provide that name if applicable.
- Indicate if you have ever been found in violation of any section of the Cincinnati Municipal Code and provide an explanation if the answer is yes.
- Explain the security and safety plans prepared for your establishment.
- Indicate the date(s) or day(s) of dance and the location of the dance event. Include the opening and closing hours.
- Sign the application, confirming that the statements made are true. Ensure you have the presence of a notary public when signing.
- Submit your application along with a self-addressed stamped envelope and payment via check or money order made payable to the City of Cincinnati.
