12. Check all jurisdictions where you operate and indicate where you maintain bulk fuel storage by marking the appropriate boxes.
13. Count the total number of vehicles that require IFTA decals and fill in that number on the form.
14. After filling out all the necessary information, review the form for accuracy. The application must be signed, or it will be returned.
