- Indicate whether you maintain bulk fuel storage in any jurisdiction and provide the distributor’s name and address if applicable.
- Check all jurisdictions where you operate and indicate where you maintain bulk fuel storage by marking the appropriate boxes.
- Count the total number of vehicles that require IFTA decals and fill in that number on the form.
- After filling out all the necessary information, review the form for accuracy. The application must be signed, or it will be returned.
- Finally, you may save changes, download, print, or share the completed form according to your needs.
