9. In the 'Applicant’s Declaration' section, read through the declaration, and ensure you understand the terms. Then print your name, sign the form, and provide the date.
10. Once you have filled out all relevant fields, review your entries for accuracy. You can then save your changes, download the completed form, print it for your records, or share it as needed.
