- Provide the total number of poultry you own over and under four months, and the date of the last Pullorum-Typhoid test, including the name of the tester. Remember to attach the required test report.
- Review and check off each requirement in the flock owner agreement to ensure compliance.
- Select the status you are applying for by checking the appropriate emblem (Bronze, Silver, Gold, or Elite).
- Sign and date the form after confirming that you understand and will comply with all requirements.
