- Complete the Primary Family Details section, capturing information for Adult A (the primary carer) and Adult B. This includes names, occupations, contact details, and the highest level of education completed.
- Fill out the Primary Family Contact Details for both adults, providing business and home telephone numbers, and preferred methods of contact.
- If applicable, provide the Primary Family Mailing Address and details of the Primary Family Doctor, including their name, address, and telephone number.
- Complete the Primary Family Emergency Contacts and Billing Address sections. If they are the same as the Family Home Address, quote 'As Above.'
- In the Demographic Details of Student section, indicate the student's date of birth, country of birth, and residency status. Provide details on language spoken at home and whether the student is of Aboriginal or Torres Strait Islander origin.
- Fill out the School Details, providing the date of first enrolment in an Australian school and any previous school information.
- If applicable, provide details about conditional enrolment as well as any student access or activity restrictions.
