This guide provides a straightforward approach to completing the Registration Form for the MAG Challenge. Clear instructions ensure that participants can easily navigate each section to provide accurate and necessary information.

Follow the steps to successfully complete your registration form.

1. Click the ‘Get Form’ button to obtain the registration form and open it in the editor.
2. Fill in the 'Name of Challenge' section with 'Trek Kilimanjaro'. Specify the 'Date of Challenge' and your 'Fundraising Target', selecting either Option 1 (£3400) or Option 2 (self-funding with no minimum target).
3. Complete the 'Personal Details' section, ensuring that you provide information as it appears on your passport. This includes your title, forename, surname, age, date of birth, email, telephone numbers, address, passport number, place of issue, date of expiry, and nationality.
4. Enclose one recent passport-sized photograph with your name printed clearly on the back.
5. In the 'Additional Information' section, answer questions regarding accommodation preferences, dietary requirements, trekking experience, and how you learned about this challenge. Don’t forget to select your t-shirt size.
6. Read the 'Conditions of Entry & Declaration' carefully. By signing this section, you agree to the stated conditions. Also, ensure that you enclose the required registration fee of £299, a completed medical questionnaire, and the passport photo.
7. Complete the 'Medical History' questionnaire with full accuracy and describe any medical conditions as prompted.
8. For those over 60 years of age or who answered 'yes' to medical questions, ensure that the next section is signed by a qualified doctor, confirming their fitness to participate.
9. Once all sections are filled accurately, review your form for completeness and correctness. Ensure that you have enclosed all necessary documents.
10. Submit your completed registration form by returning all forms to Lesley Achim at MAG, either by post or email.
