This guide provides detailed instructions for completing the National Board for Certification in Occupational Therapy, Inc. (NBCOT®) Occupational Therapist Eligibility Determination (OTED) form online. It aims to support users in efficiently navigating the form and fulfilling the necessary requirements for occupational therapy certification.
Follow the steps to complete the Nbcot Oted form online.
- Press the ‘Get Form’ button to access the OTED form and open it in the online platform.
- In the ‘OTED Applicant’ section, fill in your first name, middle name, and last name. If applicable, list any previous names you have used.
- Provide authorization by signing and dating the section that allows your college or university to share requested information with NBCOT.
- In the ‘Program Director’ section, the program director must verify that you are applying to take the NBCOT Certification Examination. They should fill in their details, including the Occupational Therapy Program name, department, college/university, address, city, state/province, country, postal code, phone number, and email.
- Skip the clinical/fieldwork experience grid if you are from a US or Canadian program. Otherwise, enter the total number of hours for full-time and part-time clinical/fieldwork experiences in the provided sections.
- Describe the type of clinical experience you have accumulated (e.g., physical disabilities, pediatrics, mental health, etc.).
- Indicate whether you completed your clinical/fieldwork under the supervision of a qualified occupational therapist. If not, provide an explanation on the reverse side.
- Fill in the total number of clinical/fieldwork hours/weeks completed and the date of completion.
- Select the appropriate accreditation option regarding your OT program at the time of graduation.
- The program director must attest to the accuracy of their responses by signing and dating the final section.

You may have already heard this, but the Therapy Ed tests are HARD. I would even say harder than the actual exam. So, if you're scoring in the 50's and 60's, don't be alarmed, because almost everyone is scoring in the 60's even right before test time. Just a heads-up: reading this book can be a bear.
