“Annual Reconciliation of School District Income Tax Withheld” (Ohio SD 141). Employers must also submit Ohio SD 141, which can be done electronically on the Ohio Business Gateway.

Every employer maintaining an office or transacting business within the state of Ohio and making payment of any compensation to an employee, whether a resident or nonresident, must withhold Ohio income tax.

Ohio law requires employers to collect the school district of residence for each employee by providing the Ohio form IT 4. Employers are required to withhold school district income tax from compensation for any employee who resides in a taxing school district.

This is common for an employer to not withhold local tax, if the city you work in does not have a tax; even though the city you live in does have a tax.

A “day” is equivalent to the largest amount of time spent at one worksite in a calendar, 24-hour day. An employee can only be considered to be in one municipality per calendar day. The employer is always required to withhold municipal income tax for each employee's principal place of work.

Thus, even if an employee lives in another state, if they work in Ohio, either part-time or full-time, the employer must withhold Ohio income tax from any wages the employee earns while working in Ohio.
