5. For cataract evaluations, provide the recommended refractive error outcome for both the right eye (OD) and left eye (OS). Additionally, specify the intraocular lens (IOL) preference, including options such as undetermined, multifocal, single-focus, or toric.
6. Indicate corneal stability by selecting the appropriate option for the type of contact lens wearer (e.g., soft lens wearer, RGP lens wearer) and complete any additional notes regarding multifocal IOL costs or post-operative care.
7. Document the clinical findings and any recommendations to the patient. This includes instructions on how long the patient should leave their contacts out prior to the exam.
8. Schedule the appointment by filling in the date and time for the patient’s visit to PCLI. Additionally, indicate if you would like PCLI to contact the patient for scheduling, and whether transportation assistance may be needed.
9. Review all entered information for accuracy before finalizing the form. Users can then save changes, download, print, or share the form as needed.
